Job Description:
A company is looking for a Care Advocate to support patients in
navigating their cancer care journey.
Key Responsibilities
Connect with patients to assess their health history and needs
related to cancer prevention and screening
Provide education and develop cancer screening and risk reduction
plans for patients
Build trust-based relationships with patients and coordinate their
care with external providers as necessary
Required Qualifications
3-5 years of experience in allied health, human services, or
healthcare supporting patients
Experience in counseling and working directly with patients
Strong clinical skills and a high standard for quality of care
Effective collaboration within a care team environment
Comfort with telehealth technology and digital tools
